When service is needed, please contact O.D.T. to discuss the nature of the problem.  A Service Order (SO) number will be assigned for the repair.  This number will be used to complete a data collection form (DCF). 
Both domestic and international versions of the DCF are accessible for printing below.  It is important to fill in this form as completely as possible.  Print a copy of the appropriate form, and complete it.  Please enclose a copy of the completed DCF with the shipment and mark the SO number on the outside of the shipping container.
